– Full name
– Age
– Phone number
– A little about yourself (1 paragraph only)
– List 10 favourite albums, EPs or singles released in the last 12 months
– Give a one paragraph review of a recent gig or event that you went to
– Tell them why you want to volunteer on-air at FBi Radio.
